All permanent changes to course modality will go through a Course Modification form in Curriculog. Making sure that the Supplemental Questionnaire is filled out and attached.

Subject: Extended Course Modality Supplement Deadline
Dear School Deans,
Due to COVID, we realize that departments may have some new interest in online, hybrid and hyflex modalities for course offerings. UGC has agreed to reopen the submission window for Fall 2021 courses to submit course action supplemental form to request alternative instructional delivery modes. The deadline has been extended until May 1st for Fall 2021 courses.
Please share this information with your department chairs and faculty, as appropriate. I want to express thanks to the UGC senate members for extending this deadline and creating additional opportunity for review of course modality proposals.
