About the Fellowship
The MiMentor Health Equity Fellowship, primarily funded by HCAI’s HPPP Category C Grant, is an eight-month, part-time opportunity designed for post-undergraduate students starting Fall 2026. Fellows will serve at a healthcare facility in collaboration with one of our partner sites, engaging in health equity projects that focus on clinical improvement, research, or health advocacy.

Eligibility Requirements

* Must be a California resident at the time of application.
* Must have completed an undergraduate program.
* Must be interested in pursuing a career in health care, including behavioral health.
* Must undergo an onboarding process, including a health screening, tuberculosis testing, and a background check.
* The annual flu vaccine, COVID vaccination, and booster are required per CDC guidelines.
* This program is open to anyone regardless of race, color, religion, sex, sexual orientation, gender identity, national origin, physical attributes, or protected veteran status.
Desired Criteria
Preference will be given to first-generation individuals and those who identify as being under-represented in medicine or
who are from a historically marginalized community economically, socially, and/or educationally.

Note: Housing and transportation are not provided.
Compensation
• Financial Support: The five (5) onboarded Fellows will receive a stipend remittance totaling $13,000 over the course of the Fellowship, disbursed biweekly to facilitate their ability to participate in this educational opportunity.
• Possibility of Travel Support: If funding permits, each Fellow will receive $200 in travel assistance per cohort year, provided as a one-time virtual gas card, EV charging card, or public transportation pass. This support is remitted after successful completion of 60 days at the partner site, fulfillment of at least 80% of required monthly hours (including HEF workshops), and maintaining good standing in the program.
• Possibility of Conference Attendance: Depending on timing, events, and budget allocations, there may be an opportunity for financial support for Fellows to attend a national or regional medical or health conference held in California.

Components
Time Commitment
• 20 hours per week
• Total of 640 hours over eight months, including potential weekend and evening obligations.
Health Improvement Project
All fellows must undertake a health improvement project focusing on primary care and/or behavioral health.

Fellow Responsibilities
• Health Equity Fellowship
* Community Engagement – Participate in available community projects or activities alongside your mentor, contributing to community-based initiatives aligned with the program's goals.
* Clinical Exposure – Engage in clinical shadowing and on-site care team operations, providing firsthand experience in healthcare settings.
* Research Opportunities – Pursue research based on the interests of the Fellow, with available opportunities to delve into specific areas of interest.
* Physician Shadowing – Observe and shadow a Physician during shifts, gaining insights into workflow and patient encounters, followed by debriefing sessions for reflection and learning as time allows.
* Health Equity Project Contribution – Collaborate on a Health Equity Project involving data collection, analysis, and contributing to the project's advancement.
* Symposium Participation (if/when available) – Showcase the Health Equity Project by presenting it at local or national symposiums in California, providing a platform to share insights and outcomes.
* Activity Tracking – Maintain a record of your on-site hours, participation in weekday workshops, completion of curriculum-related tasks, and overall hours dedicated to the program.
* Media Submission – Collect and submit media content (photos, videos) capturing ongoing projects and daily activities, ensuring adherence to privacy regulations (HIPAA) and site-specific rules while sharing valuable experiences with the MiMentor community.
* Virtual Workshops – Fellows will participate in 14 virtual lunch and/or dinner meetings with local health equity leaders. These sessions deepen insights into the curriculum's health equity focus and establish valuable connections within the field.

The MiMentor Health Equity Fellow at California Health Sciences University (CHSU) School of Medicine (SOM) will support 2-3 projects focused on improving health outcomes in underserved communities. These projects address social drivers of health, healthcare access, and public health challenges. Fellows will collaborate with medical students on initiatives such as health education, chronic disease management, community assessments, and social services integration.
Working closely with a preceptor and medical student teams, fellows will gain mentorship while contributing to impactful initiatives. Projects are conducted in partnership with CHSU-affiliated institutions, and fellows will have input on which projects they would like to pursue.
Key Responsibilities:
• Collaborate with 2-10 medical students on population health projects aimed at addressing health disparities and promoting community health.
• Participate in planning and executing initiatives like health education campaigns, chronic disease management, or mobile clinics in underserved areas.
• Support data collection and analysis to assess community health needs and tailor interventions.
• Assist in project coordination, including outreach and communication with community partners and stakeholders.
• Contribute to developing materials and presentations for project findings, including reports and oral presentations to relevant audiences.
Preferred Qualifications:
• Interest in population health, healthcare access, and working with diverse, underserved communities.
• Aspiration to pursue a medical career and interest in medical school.
• Ability to travel within a 2-hour radius of the CHSU campus (93611) and work in a hybrid setting.
Onboarding
The Fellow must complete a health screening, including required immunizations, TB screening, drug testing (if applicable), and, for some roles, a physical exam or N95 fit test. Details will be provided upon acceptance.
This fellowship offers an immersive experience in community health programming, healthcare outreach, and mentorship while contributing to projects that positively impact vulnerable populations.

The MiMentor Health Equity Fellow at Family HealthCare Network (FHCN) will gain hands-on experience working with various community health initiatives, clinical programs, and outreach efforts across FHCN sites, including the Fresno Food Pantry and The Book Nook in Visalia. The Fellow will report to the Directors of Special Programs and Clinical Education and play a key role in supporting FHCN’s mission to address social drivers of health and enhance healthcare access in the Central Valley.
Key Responsibilities:
• Participate in interprofessional clinical shadowing alongside Behavioral Health specialists, Nutritionists, Care Coordinators, PAs, and Physicians.
• Develop and support initiatives such as Reach Out & Read, Community Pantry programs, and food insecurity solutions.
• Assist with planning and executing community outreach events, creating promotional content, and enhancing public awareness of FHCN programs.
• Collaborate on Quality Improvement (QI) projects with medical students and residents, with opportunities to present findings at national conferences.
• Track community engagement, report on program and grant activities, and gather feedback to improve service delivery.
Onboarding & Training:
• Complete a non-employee application form and undergo an occupational health screening (including TB testing).
• Participate in a comprehensive health and safety training program specific to the role.
• Additional training may be required based on program needs and site requirements.

The MiMentor Health Equity Fellow at JWCH Institute will support the Data2Care (D2C) program, an initiative focused on patient outreach, engagement, and retention in care. Working closely with the Clinical Manager and interdisciplinary teams, the Fellow will address healthcare access barriers for at-risk populations, including individuals experiencing homelessness, those with mental health and substance use disorders, and communities disproportionately impacted by HIV.
Key Responsibilities:
• Conduct outreach and build trust with high-priority populations, including transgender individuals, IV drug users, and Black and Latinx communities.
• Engage patients who have fallen out of care, helping to navigate barriers such as housing instability, mental illness, and substance use.
• Work alongside clinical and outreach teams to coordinate care, input patient data into electronic health records (EHR) and LA County systems (HMIS, Case Watch, etc.), and ensure proper follow-up.
• Collaborate with community partners, public health organizations, and service providers to improve healthcare access and retention.
• Participate in meetings, training sessions, and mentorship opportunities to develop skills in healthcare delivery, public health, and patient advocacy.
Training & Support:

Fellows will be placed at various locations, including Escondido, National City, Oceanside, San Diego, San Marcos, Santee, and Spring Valley, and will report to the Regional Services Coordinator and Lead Coordinator.
Key Responsibilities:
• Conduct data collection and analysis in regions with high health disparities (e.g., Encanto, National City, Escondido) to identify pressing community needs.
• Plan, organize, and implement mobile clinics and health fairs in underserved areas to expand healthcare access.
• Participate in partner meetings, resident engagement sessions, and professional development training.
