[QSB-grads] Mandatory: Annual Evaluation Due July 20

It has been pointed out to me that the form I sent out was for ES and
contained several occurrences of "ES"  This is in fact true and I am
grateful for the ES group figuring out a straightforward method for annual
evaluations and Jay for making me aware of it.  I have updated the form so
that it is now about QSB instead of ES, and the updated version is
attached.  I will take either version.
