Asthma home visiting is an evidence-based intervention proven to reduce healthcare costs and improve asthma outcomes. Programs historically have had to rely on unstable grant funding to provide services, but in recent years, that pattern has begun to shift. Join PHI's<https://www.phi.org/> Regional Asthma Management & Prevention (RAMP) program<https://rampasthma.org/about-us/our-mission-approach/> and the National Center for Healthy Housing<https://nchh.org/> for a webinar featuring success stories from across the country. This session will highlight both state Medicaid policy advances and stories about local partnerships, as well as other innovations that create a groundswell for statewide change—all serving as models for new community action.



Learning Objectives: By the end of this webinar, participants will:

• Understand what progress and pathways for building systems to expand and sustain access to home-based asthma services might look like

• Identify one or more pathways that may lead toward sustainable home-based asthma services in their community or state

• Know how to access free technical assistance and tools to support efforts in their community or state.
